PASTOR'S
RECOMMENDATION
Each family is required
to have a recommendation from a church leader who knows their
family the best. This form is included in the Enrollment
Packet with instructions.

FEE
IMPORTANT NOTICE:
A non-refundable application fee of $25.00 per child must be
received for any application to be processed.
Please make sure that if you plan to use our online
application that you do not forget about this fee.
